What is palliative care at home?

Palliative care focuses on providing comfort, dignity, and quality of life for people with serious or life-limiting illnesses. It is not about giving up — it's about living as well as possible for as long as possible, while managing symptoms and providing emotional support.

Many people express a strong wish to spend their final days at home, surrounded by family, pets, and the things that matter most to them. Research shows that approximately 70% of people would prefer to die at home, yet only about 25% currently do so.

Our live-in palliative carers make it possible for your loved one to remain at home. They provide skilled, sensitive care around the clock, working alongside district nurses, hospice teams, GPs, and other healthcare professionals to ensure that pain and symptoms are managed effectively and that both the client and their family feel supported.

What our palliative carers provide

  • Pain and symptom management: Working with the medical team to ensure medication is administered correctly and symptoms are monitored and reported.
  • Personal care with dignity: Gentle, respectful assistance with washing, dressing, toileting, and positioning, always preserving dignity.
  • Emotional support: A calm, reassuring presence for both the client and their family. Someone to sit with, talk to, or simply be there.
  • Nutrition and hydration: Preparing light meals and ensuring adequate fluid intake, adapting to changing appetites and abilities.
  • Night-time care: Being available through the night for repositioning, medication, comfort, and reassurance.
  • Coordination with medical teams: Liaising with GPs, district nurses, hospice at home teams, and specialists to ensure joined-up care.
  • Family support: Giving family members the freedom to be family — to spend quality time with their loved one rather than being consumed by care tasks.
  • Practical help: Managing the household, answering the door, fielding phone calls, and handling the practical details so the family can focus on what matters.

Conditions we support

Our palliative care service supports people with a wide range of life-limiting conditions, including:

  • Cancer (all types and stages)
  • Heart failure
  • COPD and other respiratory conditions
  • Motor neurone disease
  • Advanced dementia
  • Kidney failure
  • Liver disease
  • Neurological conditions

We also provide end of life care — supporting people in their final days and hours with gentleness, skill, and deep respect. Our carers are experienced in recognising the signs that someone is nearing the end of life and in providing the comfort and support that is needed at this time.

Funding palliative care

People with a terminal diagnosis may be eligible for NHS Continuing Healthcare fast-track funding. This can be arranged quickly — sometimes within days — and covers the full cost of care. The fast-track process is designed for people whose condition is rapidly deteriorating or who are approaching the end of life.

We have extensive experience navigating the fast-track funding process and can help families access funding as quickly as possible. In many cases, we can arrange care to start while the funding application is being processed.

Other funding sources may include Marie Curie grants, Macmillan Cancer Support, local authority funding, and charitable grants. Our care advisors can guide you through the options.
