Why post-hospital care matters

Being discharged from hospital can be a daunting experience, especially for older people. After a hip replacement, a stroke, a fall, or a serious illness, coming home to an empty house with no support can be dangerous and frightening.

Research shows that approximately 1 in 5 older patients are readmitted to hospital within 30 days of discharge, often because they don't have adequate support at home during their recovery. Post-hospital live-in care dramatically reduces this risk.

Our post-hospital carers move in immediately after discharge — often the same day — to provide the round-the-clock support needed for a safe, comfortable recovery. They follow the hospital's discharge plan, manage medications, assist with physiotherapy exercises, prepare nutritious meals, and monitor for any signs of complications.

Common situations for post-hospital care

  • Hip or knee replacement: Support with mobility, exercises, wound care, and daily activities during the 6–12 week recovery period.
  • Stroke recovery: Specialist support with rehabilitation exercises, communication, personal care, and adapting to any new limitations.
  • Falls and fractures: Help with mobility, fall prevention, confidence rebuilding, and ensuring the home is safe.
  • Heart surgery or cardiac events: Monitoring medication, encouraging gentle exercise, preparing heart-healthy meals, and watching for warning signs.
  • Cancer treatment: Support during and after chemotherapy or radiotherapy, managing side effects, nutrition, and emotional wellbeing.
  • General surgery: Post-operative care including wound monitoring, medication management, and gradual return to normal activities.

What post-hospital care includes

  • Following the hospital discharge plan and recovery programme
  • Medication administration and monitoring
  • Wound care and dressing changes (where appropriate)
  • Physiotherapy and rehabilitation exercises
  • Mobility support and fall prevention
  • Nutritious meal preparation to support healing
  • Personal care — bathing, dressing, toileting
  • Accompaniment to follow-up appointments
  • Monitoring for complications and liaising with medical teams
  • Emotional support and encouragement
  • Household management and errands

Post-hospital care can be arranged for as little as one or two weeks, or for as long as recovery takes. Many families find that what starts as short-term post-hospital care naturally evolves into ongoing live-in care as they see the benefits.

Arranging care quickly

Hospital discharges often happen at short notice. We understand the urgency and can typically have a carer in place within 24–48 hours. In emergency situations, we may be able to arrange same-day care.

If you know your loved one has a planned surgery or hospital admission, we recommend contacting us in advance. This gives us time to find the perfect carer match and arrange a smooth transition from hospital to home.

Our care managers work directly with hospital discharge teams to ensure all the information we need is communicated clearly and nothing falls through the gaps.
